Basic information on the processing of personal data

Data administrator
The data administrator is Pharmacy Lab Sp. z o.o. with headquarters in Warsaw, ul. Wał Miedzeszyński 262, 04-792 Warsaw, Poland.

The administrator can be contacted via the e-mail address: office@pharmacylab.pl or in writing (address of the administrator’s seat). The administrator can be contacted in all matters related to the processing of personal data and in order to exercise the rights related to data processing.

GDPR

The protection and confidentiality of personal data are for Pharmacy Lab Sp. z o.o. with headquarters in Warsaw, ul. Wał Miedzeszyński 262, 04-792 Warsaw, extremely important and we want every person whose data we process to know that we apply appropriate data protection measures. Therefore, in connection with the implementation of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (general regulation on data protection) Pharmacy Lab Sp. z o.o. including:

  • implemented pursuant to Art. 24 sec. 1 and 2 of the GDPR, documentation on the protection of personal data, i.e. Security Policy, IT system management procedures.
  • applied the data security measures referred to in art. 5 sec. 1 point f GDPR.

In order to implement the above-mentioned measures is to ensure adequate protection of personal data processed by the Company as a data controller, concerning, inter alia, people reporting adverse effects of products, associates, contractors, job applicants and employees of the company.

Personal data may be collected outside the European Economic Area (“EEA”) as part of sales processes outside the EEA.

Your personal data is processed for the purposes of recruitment and employment, cooperation and exchange of correspondence, redress, product marketing, receiving and registering reports of an adverse effect of the product and forwarding this information to the competent office. Data may be transferred to other entities only for the above-mentioned purpose.

The company processes personal data only for the period specified in the law or as long as they are necessary to achieve the purpose of processing.

You have the right to access your personal data and receive a copy of the personal data subject to processing, rectify your incorrect data, request deletion of data (the right to be forgotten) in the event of circumstances provided for in art. 17 GDPR, requests to limit data processing in the cases specified in art. 18 GDPR, to object to data processing in the cases specified in art. 21 of the GDPR, to transfer the provided data, processed in an automated manner.

Any person who believes that their personal data is being processed unlawfully may lodge a complaint with the supervisory authority (PUODO, 2 Stawki Street, Warsaw).

CONTRACTOR

Your personal data will be processed for purposes related to correspondence and in connection with the cooperation between you and us. The legal basis for the processing of your personal data is art. 6 sec. 1 lit. f) GDPR. The legitimate interest of Pharmacy Lab Sp. z o.o. is the correct handling of communication between us and you, as well as the correct implementation of the cooperation agreement.

Providing personal data is voluntary, but it is necessary to achieve the above-mentioned purposes.

Data storage period

Your personal data will be processed during the period of cooperation and after its termination for a period of 5 years. In the case of cooperation related to the conclusion of the contract, the data will be processed for the period provided for by law, including the provisions of the Civil Code and the Accounting Act.

Data recipients

Your personal data will be transferred to other entities entrusted with the processing of personal data on behalf of and for Pharmacy Lab Sp. z o.o. Moreover, Pharmacy Lab Sp. z o.o. will share your personal data with entities providing postal and courier services; in the event of pursuing claims, the data will be made available to courts, court bailiffs, entities conducting debt collection activities and other recipients, if such an obligation will result from legal provisions.

Processing of data outside the EU

Your data may be processed outside the European Union.

Rights of the data subject

You have the right to access your data and the right to access your data, request rectification, deletion, and processing restrictions. To the extent that the basis for the processing of your personal data is the premise of the legitimate interest of the administrator, you have the right to object to the processing of your personal data. In order to exercise the above rights, please contact the data administrator. This can be done, for example, by sending an appropriate application to the address of the data controller or by e-mail to the address office@pharmacylab.pl or by contacting his representative.

Information on automated decision making, including profiling

Due to the processing of your personal data for the above purpose, there is no automated decision making, including profiling.

Information on the voluntary nature of providing data

Providing personal data for the above-mentioned purposes is voluntary, but necessary for its implementation.

CORRESPONDENCE

Purposes of processing and the legal basis for processing

Your data may be processed in order to contact a representative of Pharmacy Lab Sp. z o.o. – the legal basis for data processing is Art. 6 sec. 1 lit. f) GDPR, where the legitimate interest of the administrator is the ability to respond and contact the representative, direct marketing of the administrator’s own products and services, including for analytical purposes and profiling – the legal basis for data processing is your consent, provided that this consent has been provided by you, if you do not give your consent, your personal data will not be processed for this purpose.

You have the right to withdraw your consent at any time, this will not affect the lawfulness of the processing that was carried out on the basis of consent before its withdrawal.

Data storage period

Your personal data will be processed during the period of cooperation and after its termination for a period of 5 years. In the case of cooperation related to the conclusion of the contract, the data will be processed for the period provided for by law, including the provisions of the Civil Code and the Accounting Act. If the cooperation does not involve signing the contract, the data will be processed until you withdraw your consent to the processing of personal data for marketing purposes.

Data recipients

Your data may be transferred to entities processing personal data at the request of the administrator, including IT service providers – such entities process data on the basis of a contract with the administrator and only in accordance with the administrator’s instructions.

Processing of data outside the EU

Your data may be processed outside the European Union.

Rights of the data subject

You have the right to access your data and the right to request rectification, deletion, and processing restrictions. To the extent that the basis for the processing of your personal data is the premise of the legitimate interest of the administrator, you have the right to object to the processing of your personal data. In situations provided for by law, you have the right to object to processing for the purposes of direct marketing and profiling. To the extent that the basis for the processing of your personal data is consent, you have the right to withdraw your consent. Withdrawal of consent does not affect the lawfulness of the processing that was carried out before its withdrawal. To the extent that your data is processed on the basis of consent – you also have the right to transfer your personal data, i.e. to receive your personal data from the administrator in a structured, commonly used, machine-readable format. You can send this data to another data administrator. You also have the right to lodge a complaint with the supervisory body dealing with the protection of personal data. Providing personal data for the purpose of implementing the ordered contact with the representative is voluntary, but necessary for the implementation of this contact. Providing personal data for marketing purposes is voluntary.

RECRUITMENT

Purposes of processing and the legal basis for processing

Your data will be processed in order to carry out the recruitment process. The basis for processing by Pharmacy Lab Sp. z o.o. Your personal data for the purpose of conducting the recruitment process for the purposes of concluding a possible employment contract is art. 6 sec. 1 lit. b), art. 6 sec. 1 lit. c), art. 6 sec. 1 lit. f) GDPR in connection with joke. art. 221 of the Labor Code. The legitimate interest of the data controller expressed in art. 6 sec. 1 lit. f) consists in securing Pharmacy Lab Sp. z o.o. against any charges arising from Art. 123 of the Act of April 20, 2004 on employment promotion and labor market institutions.

The period for which the data will be stored

Pharmacy Lab Sp. z o.o. will process your personal data for the purposes related to the recruitment until the end of the recruitment process in which you participate and for archiving purposes, but not longer than for a period of 1 year from the date of submission of the application.

Data recipients

The administrator will transfer your personal data to other recipients entrusted with the processing of personal data on behalf of and for Pharmacy Lab Sp. z o.o., including, if necessary, suppliers of Pharmacy Lab Sp. z o.o. Moreover, Pharmacy Lab Sp. z o.o. will share your personal data with other recipients, if such an obligation will result from legal provisions.

Data transfer outside the EU

Your data may be processed outside the European Union as part of processes carried out outside the EEA.
